Azalea Trace

Care for people with memory impairment is available at Azalea Trace, a unique residence with 20 private rooms. Within the building are two “neighborhoods” designed for a maximum of 10 people each. This allows for more individual attention, increases security and enhances social interaction. With our limited number of residents, we are able to focus on each person’s strengths, talents and preferences while providing support and encouragement to family members.

Starting at $4,935/mo., Rent Includes:

• Private residences with half-bath
• 24-hour healthcare staff
• All utilities except personal telephone
• Housekeeping and linen service
• Personal laundry
• Scheduled transportation and a comprehensive daily activities program
• Three meals served restaurant-style each day
• Assistance with bathing, dressing and grooming as needed
• Ongoing monitoring of health needs and medication management under the direction of a licensed nurse
• Extended congregate care is available for an additional charge

We understand the special and often changing needs of our residents and believe in striving for the best possible quality of life for each individual. Assistance is given in a warm and caring manner under the direction of a licensed nurse with a staff dedicated to the well-being and comfort of each resident.
